**Job Summary:**
We are seeking a compassionate and dedicated Licensed Practical Nurse (LPN) to join our team at Harnett Woods Nursing and Rehab. The LPN will be responsible for providing high-quality care to our patients in a long-term care setting.

**Responsibilities:**
- Assisting patients with daily activities such as eating, dressing, and bathing.
- Administering medications and injections as prescribed by healthcare providers.
- Providing emotional support to patients and their families.
- Maintaining accurate patient records, including recording vital signs and medical history.
- Developing care plans and participating in patient care planning meetings.
- Communicating effectively with patients, families, and other healthcare professionals to ensure coordinated care.
- Applying and changing bandages, collecting samples for lab testing, and assisting with patient transfers.
- Preparing patients for discharge or transfers to other facilities.

**Qualifications:**
- Valid LPN license in the state of practice.
- Current CPR certification.
- Strong communication and interpersonal skills.
- Ability to work effectively in a team environment.
- Prior experience in long-term care or a related healthcare setting is preferred.
- Empathy and a patient-centered approach to care.
- Physical ability to assist with lifting and moving patients.
